Accidental Death, Dismemberment and Medical Care Insurance Policy via CDAoA

Highlights Include:

• CDAoA membership includes a $10,000 Accidental Death & Dismemberment Insurance, and, $5,000 Accident Medical / Dental Expense Insurance.
• $5,000 Accident Medical / Dental Expense benefit per person, per occurrence, with a $100 deductible.
• Accidental death benefit of $10,000 for primary, eligible dependents are covered at a reduced benefit amount
• Accidental dismemberment pays for the loss of limb or sight according to a schedule.
• Worldwide benefit can be used for accidents that occur out of the United States.

These benefits are provided at CDAoA’s expense to all active members who are enrolled for 30 or more consecutive days as added separate insurance benefits at no additional cost to members.
