If you make an online request to have a representative contact you, certain data is captured to verify and protect your identity (specifics are stated below).  We offer enrollment via telephone or by mail.  The decision to enroll for healthcare benefits, online or otherwise, is completely voluntary.

3.      Email address
4.      Current mailing address
5.      Credit Card or Checking Account Information (if enrolling online)

The Health Benefits Club reserves the right to use personal data to send information or promotions for Health Benefits Club services.  To opt-out by regular U.S. mail, please write to us at the address below.  You may also call us directly at 1-800-628-5061.  We will need your full name, mailing address and phone number to opt-out.
